Used to prevent the transmission of HIV to non-infected individuals

Must verify HIV negative status before starting treatment, and every 3 months while continuing the medication

Taken once daily as a combination tablet of emtricitabine and tenofovir disoproxil (200 mg-300 mg) in addition to safe sex practices

Emtricitabine and tenofovir disoproxil combination pill (Truvada) is used to prevent the transmission of HIV from infected individuals to uninfected individuals.

I agree that I:

am 18 years or older
have not had a hypersensitivity reaction to emtricitabine, tenofovir disoproxil fumarate, or any component of the formulation
understand this medication may cause a severe exacerbation of hepatitis B in individuals infected with hepatitis B that discontinue the product
am confirmed HIV-negative prior to starting treatment and will be tested every 3 months during use
understand it may increase the risk of bone fracture due to loss of bone density
understand it may increase the risk of kidney injury
am using this only for the purposes of preexposure prophylaxis
understand this medication may interact with medications being metabolized similarly, including other HIV medications, clarithromycin, clindamycin, amiodarone, verapamil, and orlistat
do not have a history of moderate to severe kidney (renal) impairment (CrCl <60 mL/min)
