I agree, under oath and penalty of perjury, that I:

am 18 years or older
am using this product as indicated for the treatment of hyperkeratotic conditions such as dry, rough skin, dermatitis, psoriasis, or other similar conditions
will apply the product 1 to 3 times per day as directed and ensure it is rubbed in until completely absorbed
understand that I may cover with adhesive bandage/gauze or plastic film
will avoid contact with eyes, lips, and mucous membranes and discontinue use if redness, discomfort, or irritation occurs
have been previously prescribed urea cream/ointment for the treatment of hyperkeratotic conditions by an authorized healthcare provider
understand the importance of applying the medication only to intact skin and avoiding use on irritated, infected, or open skin
will monitor for any signs of skin reactions such as redness, discomfort, or irritation and discontinue use if they occur
am aware of the potential for photosensitivity and will use skin protection and limit sun exposure during therapy and for one week after
do not have a hypersensitivity to urea or any component of the formulation (more information can be found at www.fda.gov)

Usage Instructions

- Wash hands after application unless treating the hands
