Terbinafine is an oral anti-fungal agent used to treat onychomycosis, which is a fungal infection of the nails. Oral terbinafine has been demonstrated to be the most effective treatment for the mycological cure of toenail fungal infections. Terbinafine works by inhibiting squalene epoxidase, the enzyme in fungal cells that convert squalene to squalene epoxide. This results in an intracellular accumulation of squalene and results in fungal cell death.

Typical dosing is 250 mg once daily for 6 weeks (fingernail) or 12 weeks (toenail)

have confirmation of diagnostic testing of nail or skin specimens prior to treatment of fungal nail infection as is recommended given potential toxicities of oral terbinafine therapy
am not pregnant or breastfeeding
do not have chronic or active liver disease
do not have active renal disease or impairment
understand terbinafine may cause hepatotoxicity ranging from mild and asymptomatic increased serum transaminases to hepatic failure. Terbinafine liver injury may present as hepatocellular hepatitis or cholestatic hepatitis.
Understand severe cutaneous adverse reactions (SCARs) have been reported with terbinafine therapy including Stevens Johnson syndrome/toxic epidermal necrolysis (SJS/TEN), drug reaction with eosinophilia and systemic symptoms (DRESS) and acute generalized exanthematous pustulosis (AGEP).
Understand altered taste and altered sense of smell may occur with terbinafine and severe cases have resulted in decreased food intake, weight loss, anxiety, or depression
Understand thrombotic microangiopathy (TMA), including thrombotic thrombocytopenic purpura and hemolytic-uremic syndrome, have been reported rarely with terbinafine therapy
will not take nor intend to take concurrently with doxorubicin, eliglustat, mequitazine, Saccharomyces boulardii, tamoxifen, or thioridazine

Terbinafine Medication Description
Terbinafine is a prescription oral antifungal medication used to treat onychomycosis, a fungal infection of the fingernails or toenails. It works by inhibiting squalene epoxidase, an enzyme essential for fungal cell membrane production. This disruption causes a buildup of squalene inside the fungal cell, leading to cell death and clearance of the infection.

Oral terbinafine has been shown to be one of the most effective treatments for achieving a complete mycological cure in toenail fungal infections. Treatment duration depends on whether fingernails or toenails are affected.

Usage Instructions
- Take exactly as prescribed by your healthcare provider, usually once daily
- Continue therapy for the full duration prescribed, even if nail appearance improves before treatment is complete
- Fingernail infections are typically treated for 6 weeks; toenail infections for 12 weeks
Warnings & Precautions
- Use only under medical supervision, especially if you have liver disease or a history of liver problems
- Your provider may order blood tests to monitor liver function during treatment
- Possible side effects include headache, diarrhea, stomach upset, rash, or changes in taste
- Seek medical attention if you experience persistent nausea, fatigue, dark urine, or yellowing of the skin or eyes, as these may be signs of liver damage
- Inform your healthcare provider of all medications you take, as terbinafine may interact with certain drugs
