I agree that I:

• am 18 years or older
• have not had a hypersensitivity reaction to telmisartan or any component of the formulation

• have a diagnosis of high blood pressure and have already been taking telmisartan as prescribed by an authorized healthcare provider
• Am taking telmisartan as prescribed for my own personal use 

• Do not take, nor intend to take, aliskiren concurrently with telmisartan
• do not have hepatic (liver) impairment, renal (kidney) impairment
• am not pregnant or breastfeeding and understand taking telmisartan while pregnant may cause injury and/or death to the developing fetus
• have already been taking the initial dose (telmisartan 20 mg) or have been previously prescribed telmisartan 40 mg or telmisartan 80 mg, if selecting telmisartan 40 mg or telmisartan 80 mg
• understand this medication may cause angioedema, which is characterized by sudden swelling in areas under the skin and throat closure. I will discontinue this medication and seek medical attention immediately if angioedema occurs.
• do not have the following conditions: aortic or mitral stenosis, ascites, or renal artery stenosis
• understand this medication may cause acute kidney injury and high potassium levels
• Will not take nor intend to take the following concurrently with telmisartan due to severe or major drug-drug interactions: aliskiren, amifostine, angiotensin-converting enzyme (ACE) inhibitors, bromperidol, isocarboxazid, lithium, Obinutuzumab, patiromer, ramipril, sparsentan
