I agree that I:

am 18 years or older
do not have uncorrected adrenal insufficiency
have not had a hypersensitivity reaction to levothyroxine or any component of the formulation
have a diagnosis of hypothyroidism and have been taking levothyroxine as prescribed by an authorized healthcare provider
have had my thyroid levels tested within the past 12 months and am taking this medication at the recommended dose

understand this medication may exacerbate existing adrenal insufficiency and may decrease bone mineral density if overtreated
understand this medication may interact with medications being metabolized similarly or that share similar side effects, including antacids, iron supplements, calcium supplements and bile acid sequestrants
understand the life-threatening effects of being overtreated with levothyroxine on cardiovascular health, including increased risk of arrhythmias, palpitations, and rapid heart rate

Synthroid Medication Description
Synthroid® (levothyroxine) is a prescription thyroid hormone replacement used to treat hypothyroidism, a condition where the thyroid gland does not produce enough thyroid hormone. Levothyroxine is a synthetic form of thyroxine (T4), the primary hormone made by the thyroid, which helps regulate metabolism, energy levels, and overall body function. Restoring normal thyroid hormone levels can help improve symptoms such as fatigue, weight gain, dry skin, hair thinning, and sensitivity to cold.

Synthroid® dosing is individualized and based on blood tests and clinical response. Because thyroid hormone needs can change over time, ongoing monitoring is important to ensure optimal treatment.

Usage Instructions
- Take exactly as prescribed by your healthcare provider
- Take with a full glass of water on an empty stomach
- Wait 30 to 60 minutes before eating or drinking anything other than water
- Take at the same time each day for consistent absorption
- Do not stop taking without medical guidance, even if you feel well
Warnings & Precautions
- Do not use for weight loss; high doses can be dangerous
- Inform your provider if you have heart disease, adrenal gland problems, or osteoporosis
- May interact with other medications, including certain supplements, antacids, and cholesterol-lowering drugs. Take these at least 4 hours apart from levothyroxine
- Possible side effects when overtreated include rapid heartbeat, chest pain, anxiety, sweating, or weight loss
- Seek medical attention if you experience signs of allergic reaction such as rash, swelling, or difficulty breathing

Side effects may include chest pain, rapid heart rate, headache, and dizziness.
