Sumatriptan

Sumatriptan is used for the management of acute migraine headaches with or without aura. It should not be used for more than 10 days a month or for chronic headaches.

I agree that I:

• am 18 years or older
• have not had a hypersensitivity reaction to sumatriptan or any component of the formulation
  
• have a diagnosis of migraines and have already taken sumatriptan as prescribed by an authorized healthcare provideram not pregnant or breastfeeding
• understand this medication is not recommended in those 65 years and older due to cardiovascular risk
• do not have a history of cardiovascular disease or ischemic coronary artery disease (e.g. history of heart attack (myocardial infarction), angina pectoris)
• do not have a history of Prinzmetal angina
• do not have a history of stroke or transient ischemic attack
• do not have the following conditions: peripheral vascular disease, ischemic bowel disease, uncontrolled hypertension
• do not have a seizure disorder
• do not have Wolff-Parkinson-White syndrome or other conditions associated with heart arrhythmias
• do not have hemiplegic migraine or basilar migraine
• have not used a monoamine oxidase inhibitor (MAOI) such as isocarboxazid, phenelzine, tranylcypromine, or selegiline currently or within the past 14 days
• have not taken another triptan (e.g. sumatriptan) or ergotamine (e.g. methysergide, dihydroergotamine) in the past 24 hours
• understand this medication increases the risk of life-threatening events including heart attack, stroke, cardiac arrest, splenic infarction, and death
• understand use of this medication for more than 10 days per month may worsen headaches
• understand this medication may cause vision loss or abnormalities
• do not have hepatic (liver) impairment, renal (kidney) impairment
• understand this is not a comprehensive list of precautions, risks, side effects, or details about taking this medication
• understand that only major drug interactions are listed and have not been reviewed against the other medications, supplements, or herbal products I may be taking unless also filled by TelyRx

• understand that TelyRx LLC does not provide medical care and the providers who use TelyRx to provide telehealth services (“Telehealth Providers”) should not replace your primary provider.
• understand that the Telehealth Providers are not able to: (1) monitor ongoing health or lab values for side effects or efficacy, (2) observe how you (or your pet) tolerate any prescribed treatment, or (3) determine if the prescribed treatment is working as intended and assume any risks associated with this modality of care and waive any claim I may have in the future against TelyRx and/or the Telehealth Providers.
• understand that by using this site, I have created a patient-client-doctor relationship and I am being directed by the Telehealth Providers to consult my primary provider soon after beginning any treatment prescribed by the Telehealth Providers.
