Mounjaro is injected under the skin once weekly and may be increased based on clinical response and tolerance. have not had a hypersensitivity reaction to Mounjaro or any component of the formulation
do not have a personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN2)
do not have Type 1 diabetes or diabetic ketoacidosis (DKA)
am not taking a dipeptidyl peptidase-4 inhibitor, such as sitagliptin (Januvia) or saxagliptan (Onglyza)
understand psychiatric effects have occurred, such as suicidal thoughts, with other medications for weight management
have not had bariatric surgery
understand this medication may slow gastric emptying which can alter the absorption of other medications
understand this medication has shown incidence of acute kidney injury, diabetic retinopathy, gallbladder disease, gastrointestinal side effects, and pancreatitis
am not pregnant or breastfeeding
