I agree that I:

am 18 years or older
have not had a hypersensitivity reaction to metoprolol or other medications within the same class (beta blockers e.g. atenolol, bisoprolol, carvedilol, nadolol, propranolol), or any component of the formulation
do not have second- or third-degree heart block, or significant first-degree heart block
do not have any of the following conditions; severe slow heart rate (bradycardia), severe sinus bradycardia (heart rate <45 bpm), cardiogenic shock, overt heart failure, sick sinus syndrome, severe peripheral arterial circulatory disorders, moderate heart failure, or a systolic blood pressure of <100 mm Hg
understand that the medication must be gradually discontinued, and I cannot stop taking metoprolol abruptly due to the risk of recurring chest pain (angina pectoris) and heart attack (myocardial infarction)
understand this medication may interact with medications being metabolized similarly, including diphenhydramine, duloxetine, furosemide and levothyroxine
have a diagnosis of high blood pressure and have already been taking metoprolol tartrate as prescribed by an authorized healthcare provider
understand this medication may cause too low blood pressure and dizziness, which must be monitored, and dose adjusted as needed
do not have severe liver (hepatic) impairment
do not have Prinzmetal variant angina or supraventricular tachycardia (SVT)
understand this medication may mask symptoms of diabetes and overactive thyroid disease
am not pregnant or breastfeeding
understand this medication may worsen symptoms of asthma or COPD

How To Use:
