Ivermectin - Scabies

Oral ivermectin is used to treat scabies for individuals that have had prolonged skin to skin contact to scabies within the past 6 weeks. It may also be used for scabies infestation for individuals living in the same environment. This formulation may also be used in combination with topical treatments for severe or difficult infestations.

am 18 years or older
Understand that Ivermectin is contained in a non child resistant foil pack
I request that I receive Ivermectin in it's original container and consent to receiving it in this non child resistant manner
have not had a hypersensitivity reaction to ivermectin or any component of the formulation
am not pregnant or breastfeedingam using oral ivermectin for the treatment of scabies infestation
am not on dialysis or have severe kidney dysfunction
understand oral ivermectin can affect the central nervous and present as side effects including confusion, tremor, impaired consciousness, and breathing difficulties
understand oral ivermectin can cause a delayed hypersensitivity reaction and severe skin reactions such as Stevens-Johnson Syndrome (SJS) and toxic epidermal necrolysis (TENS)
understand this is not a comprehensive list of precautions, risks, side effects, or details about taking this medication
understand that only major drug interactions are listed and have not been reviewed against the other medications, supplements, or herbal products I may be taking unless also filled by TelyRx
