have a diagnosis of high blood pressure or fluid retention and have already been taking hydrochlorothiazide as prescribed by an authorized healthcare provider
do not have anuria
understand this medication may cause increased urination and electrolyte depletion
have not had bariatric surgery
do not have ascites from liver cirrhosis, Addison’s disease, or systemic lupus erythematosus (SLE)
understand this medication may interact with medications being metabolized similarly, including lithium, phenobarbital, and non-steroidal anti-inflammatory drugs (NSAIDs e.g. ibuprofen)
do not have diabetes
do not have severe liver (hepatic) or severe kidney (renal) impairment (CrCl <30 mL/min)
am not taking lithium
am not pregnant or breastfeeding
understand this medication may cause light sensitivity, severe fluid and electrolyte loss, worsen gout, hypersensitivity reactions, and vision changes

Medication Description

Hydrochlorothiazide is a thiazide diuretic commonly prescribed to lower blood pressure and reduce excess fluid in the body. It works by promoting the excretion of water and electrolytes—such as sodium, chloride, and potassium—through the kidneys. This reduces blood volume, helping to control hypertension and relieve swelling (edema) caused by conditions like heart failure or liver disease.

Initial daily dosing typically starts between 12.5 mg and 25 mg, with many patients maintaining a 25 mg dose. Older adults often respond well to lower doses (e.g., 12.5 mg), though close monitoring of electrolyte levels is recommended to avoid imbalances. The medication is generally taken in the morning with or without food to minimize sleep disruption from increased urination.

How To Use:

Reduces blood pressure and treats mild swelling (edema)

Common dose: 12.5 mg–25 mg once daily

May cause electrolyte imbalances, especially potassium loss

Monitor for signs of dehydration or dizziness

Recommended for morning use to avoid nighttime urination
Side Effects

Side effects may include dizziness, headache, upset stomach, decreased appetite, light sensitivity, and stomach cramps. This medication may interact with nonsteroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen, naproxen, and aspirin. Avoid using NSAIDs in combination with hydrochlorothiazide as it may decrease its effectiveness.
