I agree that I:

am 18 years or older
have not had a hypersensitivity reaction to fenofibrate or any component of the formulation
do not acute liver failure or decompensated cirrhosis
have a diagnosis of high cholesterol and have already been taking fenofibrate as prescribed by an authorized healthcare provider
am only taking this medication with a statin drug as previously prescribed by an authorized healthcare provider
do not have liver (hepatic) impairment (Child-Pugh class A-C)
do not have severe kidney (renal) impairment, end stage kidney disease or am receiving dialysis
understand it may increase blood sugar levels
am not pregnant or breastfeeding
do not have gallbladder disease
understand this medication may interact with medications being metabolized similarly, including cyclosporine, warfarin, bile acid sequestrants, and leflunomide
understand it may increase the risk of hepatotoxicity
understand it increases the risk of muscle weakness, including myalgia, myopathy, and rhabdomyolysis. If muscle pain or severe weakness occurs, I will discontinue the medication and seek medical attention immediately.
understand this medication increases the risk of gallstones and will discontinue this medication if gallstones are present
understand this medication can severely decrease the levels of good cholesterol (HDL-C)
understand this medication may increase the risk of blood clots (e.g. pulmonary embolism, deep vein thrombosis)
do not have an allergy to peanuts, soy, or arachis

How To Use:
