Cyanocobalamin is used to treat or prevent vitamin B12 deficiency caused by dietary deficiencies, certain types of anemias, pregnancy, and other disease states. Cyanocobalamin injections for maintenance therapy are conveniently dosed just once a month and are injected into the muscle or deep under the skin.

Cyanocobalamin (Vitamin B12) Injection

I agree that I:

Am 18 years or older
Have not had a hypersensitivity reaction to cyanocobalamin (vitamin B12), cobalt, or any component of its formulation
Do not have a history of hypokalemia (low potassium)
Do not have a history of Leber disease
Understand this medication is administered into the muscle (IM) or deep under the skin (SubQ) only and is not to be used intravenously (IV)
Understand that vitamin B12 is not a substitute for folic acid. Patients with megaloblastic anemia may also require folic acid supplementation as directed by a licensed healthcare provider
Do not have a history of polycythemia vera
Understand that treatment of severe vitamin B12 megaloblastic anemia with cyanocobalamin may result in thrombocytosis (high platelet counts) and an increased risk for blood clots or bleeding
Understand this formulation may contain aluminum and although rare, toxic aluminum concentrations have been seen with high doses, prolonged use or kidney impairment
Do not heavily consume alcohol (>2 weeks) and understand that heavy alcohol consumption may impair absorption of vitamin B12

Medication Description
Cyanocobalamin is a form of vitamin B12 used to treat or prevent B12 deficiency, which can result from dietary limitations, certain types of anemia, pregnancy, gastrointestinal disorders, or other medical conditions. Vitamin B12 is essential for healthy nerve function, red blood cell formation, and DNA synthesis.

This prescription-strength B12 injection is typically used as a maintenance therapy and is often dosed at 1 mL once a month. It is injected intramuscularly or subcutaneously (deep under the skin), depending on the provider’s instructions and patient preference.

Self-administration is common and can be done into the upper, outer thigh. If someone else is giving the injection, it may also be administered into the deltoid muscle (upper arm) or the gluteal muscle.

How to Use:
- Typical dosage is to inject 1 mL into the muscle or under the skin once per month
- Use a clean needle and syringe for each injection
- Follow your healthcare provider’s directions closely
Warnings:
- Not for individuals with hypersensitivity to cobalt or vitamin B12
- Use with caution in patients with Leber’s disease or other optic nerve disorders
- In rare cases, high doses may lead to low potassium levels or allergic reactions
