Nebivolol

I agree that I:

• Am 18 years or older
• Will use this medication for my own personal use and as indicated 
• Am currently taking this medication as prescribed by an authorized healthcare provider based on having a diagnosis of high blood pressure 
• Have not had a hypersensitivity (allergic) reaction to nebivolol, any other beta blockers, or any component of the nebivolol formulation (More information about formulation components can be found at www.drugs.com)
• Understand the maximum daily dose is 40mg per day
• Understand nebivolol is not recommended for hypertension in the absence of specific comorbidities (eg, ischemic heart disease, heart failure with reduced ejection fraction, arrhythmia)
• Do not have liver (hepatic) or kidney (renal) impairment
• Do not have a history of severe bradycardia and understand that nebivolol is contraindicated with this condition
• Do not have a history of the following and understand the conditions are contraindicated when taking nebivolol: heart block greater than first degree, cardiogenic shock, decompensated cardiac failure, sick sinus syndrome
• Do not have a history of anaphylactic reactions and that treatment of anaphylaxis in patients taking beta-blockers may be ineffective or promote undesirable effects
• Understand that beta-blockers may mask early warning signs of hypoglycemia and increase the risk of severe or prolonged hypoglycemia
• Do not have a history of bronchospastic disease
• Do not have a history of Myasthenia gravis
• Do not have a history of Peripheral vascular disease (PVD) and/or Raynaud disease
• Do not have untreated Pheochromocytoma
• Do not have a history of psoriasis
• Do not have a history of thyroid disease
• Do not have a history of vasospastic angina
• Understand that therapy should not be abruptly withdrawn, but gradually tapered to avoid acute tachycardia, hypertension and/or ischemia
• Understand that chronic beta-blocker therapy should not be routinely withdrawn prior to major surgery
• Am not pregnant or breastfeeding
• Understand that typical monitoring parameters include blood pressure, heart rate, ECG, glucose (in patients with diabetes), mental alertness and signs/symptoms of bronchospasm
• Understand that the following medications have significant drug interactions and am not currently taking: amifostine, bromperidol, ceritinib, etofylline, fexinidazole, fingolimod, grass pollen allergen extract, isocarboxazid, mavorixafor, obinutuzumab, patiromer, ponesimod, rivastigmine, siponimod, tasimelteon, or white birch allergen extract
• Understand and am aware that like many beta blockers, nebivolol has a risk of significant adverse events including but not limited to the following: bradyarrhythmias, bronchospasms, CNS effects, potentiation/masking of hypoglycemia, and abrupt withdrawal
