Carvedilol

I agree that I:

• am 18 years or older
• have not had a hypersensitivity reaction to carvedilol or any component of the formulation

• have a diagnosis of high blood pressure and/or heart failure and already been taking carvedilol as prescribed by an authorized healthcare provider 
• Am taking carvedilol as prescribed for my own personal use 

• do not have a history of the following heart conditions: decompensated cardiac failure requiring intravenous inotropic therapy, bronchial asthma or related bronchospastic conditions, second or third degree atrioventricular (AV) block, cardiogenic shock, sinus bradycardia, sick sinus syndrome, or vasospastic angina
• do not have severe hepatic impairment
• do not have second- or third-degree heart block, or significant first-degree heart block
• understand beta-blocker therapy, such as carvedilol, should not be withdrawn abruptly, but gradually tapered to avoid severe exacerbation of chest pain, the occurrence of heart attack, ventricular arrhythmias, acute tachycardia, high blood pressure, and/or ischemia in patients with underlying cardiovascular disease.
• do not have hepatic (liver) impairment, renal (kidney) impairment
• am not pregnant or breastfeeding
• have already been taking the initial dose (carvedilol 3.125 mg) or have been previously prescribed carvedilol 6.25 mg, carvedilol 12.5 mg, or carvedilol 25 mg if selecting carvedilol 6.25 mg, carvedilol 12.5 mg, or carvedilol 25 mg
• understand symptomatic hypotension with or without syncope may occur with carvedilol (usually within the first 30 days of therapy)
• understand carvedilol may aggravate the following medical conditions and thus must be used with caution or avoided: asthma, chronic obstructive pulmonary disease (COPD), diabetes, myasthenia gravis, peripheral vascular disease, untreated pheochromocytoma, psoriasis, renal impairment, thyroid disease, vasospastic angina
• understand beta-blockers may cause bradycardia or first, second, or third-degree atrioventricular (AV) heart block, or complete atrioventricular block
• understand nonselective beta-blockers (eg, carvedilol) have a higher risk of bronchospasm compared to cardioselective beta-blockers and may lead to drug discontinuation in patients with chronic obstructive pulmonary diseases (COPD) or asthma
• Understand beta-blockers may cause reversible CNS effects such as fatigue, insomnia, vivid dreams, memory impairment, and sexual disorder
• Understand beta-blockers may worsen, prolong, or cause hypoglycemia (ie, low blood sugar). Additionally, beta-blockers may mask symptoms of hypoglycemia (tremor, irritability, palpitations), making diaphoresis (ie, excessive sweating) the only symptom unaffected by beta-blockers.
• Will not take nor intend to take the following concurrently with carvedilol due to severe or major drug-drug interactions: afatinib, alpha-2 agonists, amifostine, beta-2 agonists, bilastine, bromperidol, ceritinib, colchicine, dronedarone, etofylline, fexinidazole, fingolimod, grass pollen allergen extract, isocarboxazid, Lefamulin, mavorixafor, Obinutuzumab, patiromer, pazopanib, ponesimod, pralsetinib, relugolix, repotrectinib, Rimegepant, rivastigmine, Siponimod, sirolimus, talazoparib, tasimelteon, topotecan, Ubrogepant, venetoclax, vincristine, white birch allergen extract
• Understand concurrent use of carvedilol with amiodarone, digoxin, or verapamil may cause heart block, bradycardia, and/or left ventricular dysfunction
