Zolmitriptan is an antimigraine agent and belongs to a class of medications known as “triptans”. Zolmitriptan works on serotonin receptors and is intended to relieve the symptoms of an active migraine rather than prevention. When compared to other triptans (sumatriptan and rizatriptan), zolmitriptan has fewer risks of side effects and less known significant drug interactions.

Zolmitriptan is used for the acute treatment of moderate to severe migraines with or without aura in adults

Zolmitriptan is not intended to be used for prevention of migraines

Zolmitrpitan is not intended to be used for the treatment of cluster headaches

Zolmitriptan

I agree that I:

Am 18 years or older
Will be using this medication as prescribed and for my own personal use
Have not had a hypersensitivity reaction to zolmitriptan or any component of the formulation
Have had a clear diagnosis of migraine and will only use this medications for acute treatment
Will not use this medication for the prevention of migraines
Will not use this medication within 24 hours of an ergotamine preparation or a different triptan (E.g. sumatriptan, rizatriptan)
Will limit use to less than 10 days per month to avoid medication overuse headache
Understand and am aware that the maximum dose is 5mg per dose; and 10mg per 24 hours and will not exceed the maximum dose
Do not have a history of renal (kidney) impairment
Do not have a history of liver disease and/or do not have a history of impaired liver function
Do not have a history of ischemic coronary artery disease, coronary artery vasospasm, or other significant underlying cardiovascular disease
Do not have a history of Wolff-Parkinson-White syndrome or arrhythmias associated with other cardiac accessory conduction pathway disorders
Do not have a history of peripheral vascular disease
Do not have a history of ischemic bowel disease
Do not have a history of uncontrolled hypertension
Do not have a history of stroke, transient ischemic attack, or hemiplegic migraine or migraine with brainstem aura
Will not use a monoamine oxidase A (MAO A) inhibitor within 2 weeks of using zolmitriptan
Do not have a history of cardiac events, cerebrovascular events, elevated blood pressure, vasospasm related events
Understand and am aware of the potential risk of visual effects (partial vision loss and blindness have been reported with this class of medications)
Am not pregnant or breastfeeding
Understand and am aware that significant drug interactions exist for the following drugs or drug classes, and that I am not taking any of the following: bromocriptine, cimetidine, droxidopa, ergot derivatives, monoamine oxidase inhibitors, serotonergic agents.

Zolmitriptan side effects may include the following: Unpleasant taste, chest pain, chest pressure, facial edema, palpitations, dizziness, drowsiness, diaphoresis and nausea.
