Zafirlukast is used for the chronic treatment of asthma when other agents have failed to control persistent asthma alone. This medication is suitable for individuals that have already been taking zafirlukast as prescribed by an authorized healthcare provider.

Used as a maintenance therapy for persistent asthma that cannot be controlled with an inhaled corticosteroid

Works by decreasing inflammation in the airways, making it easier to breathe

To be taken on an empty stomach either 1 hour before or 2 hours after meals

Must be taken as prescribed for at least 1 to 2 months before assessing efficacy

I agree that I:

am 18 years or older
have not had a hypersensitivity reaction to zafirlukast or any component of the formulation

have a diagnosis of asthma and have already been taking zafirlukast as prescribed by an authorized healthcare provider
Am taking zafirlukast as prescribed for my own personal use

do not have hepatic (liver) impairment including hepatic cirrhosis
am not pregnant or breastfeeding
do not have phenylketonuria (PKU)
understand zafirlukast must be taken daily and is not for use of acute asthma attacks
understand severe hepatic adverse events (including hepatitis, hyperbilirubinemia, and hepatic failure) have been reported with use of zafirlukast and to be alert for and immediately report symptoms of anorexia, right upper quadrant abdominal pain, and nausea to my primary care provider
understand postmarketing reports of behavioral changes (ie, depression, insomnia) have been noted with zafirlukast and will report neuropsychiatric symptoms/events during therapy to my primary care provider
understand zafirlukast is not approved for use in the reversal of bronchospasm in acute asthma attacks, including status asthmaticus; therapy can be continued during acute exacerbations of asthma
understand concomitant use of zafirlukast with warfarin results in a clinically significant increase in INR
have already been taking the initial dose (zafirlukast 10 mg) or have been previously prescribed zafirlukast 20 mg if selecting zafirlukast 20 mg
understand zafirlukast is associated with an increased incidence of respiratory tract infections, particularly in patients aged 55 or older that concomitantly take inhaled corticosteroids
Will not take nor intend to take the following concurrently with zafirlukast due to severe or major drug-drug interactions: loxapine, systemic erythromycin, phenytoin, theophylline, tolbutamide, warfarin

Zafirlukast Side Effects

Zafirlukast side effects may include: headache, dizziness, abdominal pain, diarrhea, nausea, vomiting, dyspepsia, back pain, weakness, depression, insomnia, abnormal dreams, hallucinations, elevation in liver enzymes; respiratory tract infection in patients >55 years old with coadministration of inhaled corticosteroids.
