Dosing begins at 0.25 mg once weekly for 4 weeks then increased to 0.5 mg weekly

Ozempic® is used to aid in weight loss when used in combination with diet and exercise. Ozempic® is injected under the skin once weekly and may be increased based on clinical response and tolerance. Follow package instructions upon receiving the product.

am 18 years or older
have not had a hypersensitivity reaction to Ozempic®, semaglutide, or any component of the formulation
I have taken and tolerated the previous dosing tiration (if applicable)
do not have a personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN2)
do not have Type 1 diabetes or diabetic ketoacidosis (DKA)
understand that the use of Ozempic® for weight loss is not approved by the FDA and serves as an off-label use
understand psychiatric effects have occurred, such as suicidal thoughts, with other medications for weight management
have not had bariatric surgery
understand this medication may slow gastric emptying which can alter the absorption of other medications
understand this medication has shown incidence of acute kidney injury, diabetic retinopathy, gallbladder disease, gastrointestinal side effects, and pancreatitis
am not pregnant or breastfeeding
do not have liver (hepatic) or kidney (renal) impairment
