Clobetasol Cream

I agree that I:

• am 18 years or older
• have not had a hypersensitivity reaction to clobetasol or any component of the formulation or other corticosteroids
• have already been using clobetasol as prescribed by a dermatologist or an authorized healthcare provider
• am not pregnant or breastfeeding
• will discontinue the use of this product if local sensitivity occurs
• will contact my provider if symptoms remain for 14 days or longer after use
• will use the medication as prescribed for the appropriate treatment and duration
• will not use clobetasol if atrophy is present at the treatment site
• will not use clobetasol with occlusive dressing or cover, bandage, or wrap the treated area unless instructed by an authorized healthcare provider
• will not use clobetasol on the face, eyes or eyelids, axillae, groin, or intertriginous areas
• will not use clobetasol for the treatment of acne vulgaris, rosacea, perioral dermatitis, perianal or genital pruritus
• understand the topical formulation of this product may cause adrenal suppression, allergic contact dermatitis, folliculitis, acneiform eruptions, hypopigmentation, perioral dermatitis, secondary infection, striae, miliaria, skin atrophy, or telangiectasia
• understand the topical formulation of this product, is unlikely to, but may cause systemic effects (e.g. Cushing syndrome, high blood sugar, high sugar levels in urine) if used for a prolonged period of time, applied to large areas, applied to denuded skin, or wrapped in occlusive dressing
• understand topical corticosteroids including clobetasol may increase the risk of posterior subcapsular cataracts and glaucoma and contact with eyes is to be avoided
• understand clobetasol is for topical use only and is not for oral, ophthalmic, or intravaginal use
• understand that topical corticosteroids such as clobetasol are not recommended for bacterial infections as their anti-inflammatory and vasoconstrictive effect will mask the infection, ultimately delaying diagnosis and treatment
• understand clobetasol foam and spray are flammable and fire, flame, or smoking will be avoided during and immediately following application
