Wegovy - 1MG - 1 Box (4 Injectable pens)

Wegovy is used for chronic weight management in individuals with a BMI of > 30 kg/m2, or a BMI of > 27 kg/m2 and weight associated comorbidities, including high cholesterol, diabetes, and high blood pressure. Wegovy is injected under the skin once weekly with a total duration of therapy of up to 16 weeks. Therapy can be discontinued soon if desirable results are achieved. Follow package instructions upon receiving the product.

I agree that I: 

• am 18 years or older 
• have not had a hypersensitivity reaction to Wegovy, semaglutide, or any component of the formulation 
• do not have a personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN2)
• do not have Type 1 diabetes or diabetic ketoacidosis (DKA)
• understand psychiatric effects have occurred, such as suicidal thoughts, with other medications for weight management
• have not had bariatric surgery
• understand this medication may slow gastric emptying which can alter the absorption of other medications
• understand this medication has shown incidence of acute kidney injury, diabetic retinopathy, gallbladder disease, gastrointestinal side effects, and pancreatitis
• am not pregnant or breastfeeding
• do not have liver (hepatic) or kidney (renal) impairment
