Estradiol (estrogen) is a female hormone used to help alleviate symptoms of menopause, including vaginal dryness, burning, and painful intercourse. Estradiol oral tablets are FDA-approved to treat vulvar and vaginal atrophy associated with menopause. Vaginal atrophy (atrophic vaginitis) is thinning, drying and inflammation of the vaginal walls that may occur when your body has less estrogen. They’re also FDA approved to treat vasomotor symptoms associated with menopause.

Used to help reduce symptoms of menopause, such as vaginal dryness, burning, and painful intercourse

Must be 60 years of age or younger and within 10 years of menopause

Typical maintenance dosing is 0.5mg to 1mg daily

I agree that I:

am 18 years or older
am less than 60 years of age or within 10 years of menopause
have not had a hypersensitivity reaction to estradiol or any component of the formulation
am using this treatment as intended for menopause
do not have abnormal genital bleeding
do not have migraine headaches with aura
do have a current or history of blood clots, including venous thromboembolism (VTE) or deep vein thrombosis (DVT), or arterial thromboembolic disease such as stroke or heart attack
do not have a history of breast cancer or estrogen dependent tumor
do not have hepatic (liver) impairment or disease
do not have a blood disorder, including deficiencies in protein C, protein S, antithrombin, or other blood clotting diseases
understand, am aware and that I am not taking nor intend to take this medication with medications being metabolized similarly, including phenobarbital, phenytoin, rifampin, and St. John’s Wort
understand, am aware and that I am not currently taking nor intend to take this medication with anastrozole, cosyntropin, dehydroepiandrosterone, exemestane, fexinidazole, fezolinetant, fusidic acid (systemic), growth hormone analogs, hemin, metyrapone, ospemifene, raloxifene, tizanidine, transexamic acid due to the major drug-drug interactions
am not pregnant or breastfeeding
understand this medication may increase the risk of endometrial cancer and/or exacerbate endometriosis, and increase the risk of ovarian cancer and invasive breast cancer
understand this medication may increase risk of DVT, pulmonary embolism, stroke, or myocardial infarction in postmenopausal women
understand this medication may increase the risk of pancreatitis, cholelithiasis, and cholecystitis
understand this medication may increase the risk of dementia
understand this medication may increase the risk of blood clots in the retina (retinal vascular thrombosis) and will discontinue if experiencing migraine, loss of vision or other vision changes
understand this medication may worsen other disease states or conditions (e.g. asthma, cardiovascular disease, diabetes mellitus, epilepsy, and migraine)
have previously been prescribed and am still currently taking the requested dose (0.5mg, 1mg or 2mg)
understanding smoking cigarettes while taking this therapy will significantly increase risk of blood clot
