Finasteride Tablets (BPH)

I agree that I:  

• am 18 years or older  

• have not had a hypersensitivity reaction to finasteride or any component of the formulation  

• have already been taking finasteride as prescribed by an authorized healthcare provider 

• Am taking finasteride as prescribed for benign prostatic hyperplasia (BPH) for my own personal use 

• am not pregnant, planning to become pregnant, or breastfeeding  

• do not have liver (hepatic) or kidney (renal) impairment  

• understand this medication may cause sexual dysfunction, including impotence, decreased libido, ejaculatory disorder, and sexual disorder, which may persist after discontinuation 

• Understand finasteride reduces prostate specific antigen (PSA) concentration by ~50% within 6 months of treatment and a new PSA baseline would need to be reestablished after initiation of therapy 

• Understand finasteride is reserved for use in patients with significantly enlarged prostates (prostate volume >30 ml, prostate-specific antigen >1.5 ng/ml, or palpable prostate enlargement on digital rectal exam) or those with hematuria (blood in urine) associated with benign prostatic hyperplasia 

• Understand patients who could become pregnant should not touch or handle crushed or broken tablets of finasteride 

• Understand finasteride may affect male fertility; male infertility and poor seminal quality have been reported and may be reversible upon discontinuation of finasteride
