Ketoconazole is an oral anti-fungal agent used to treat systemic fungal infections.

Ketoconazole treats susceptible systemic fungal infections including blastomycosis, histoplasmosis, paracoccidioidomycosis, coccidioidomycosis, and chromomycosis in patients who have failed or who are intolerant to other antifungal therapies. Ketoconazole tablets are not indicated for the treatment of onychomycosis, cutaneous dermatophyte infections, Candida infections or fungal meningitis.

Suitable in patients already taking this therapy under supervision of their medical doctor or provider

Ketoconazole tablets should only be used when other effective antifungal therapy is not available or tolerated and the potential benefits outweigh the potential risks

Typical dosing is 200 mg by mouth daily until active fungal infection is resolved; may increase to 400 mg once daily if response is insufficient

To be taken at least 2 hours before any antacids

have already been using oral ketoconazole therapy as prescribed by an authorized healthcare provider
am not pregnant or breastfeeding
do not have a history of or current acute or chronic liver disease
do not have a history of or current adrenal insufficiency
will not take nor intend to take concurrently with dofetilide, quinidine, pimozide, lurasidone, cisapride, methadone, disopyramide, dronedarone, or ranolazine. Ketoconazole can cause elevated plasma concentrations of these drugs and may prolong QT intervals, sometimes resulting in life-threatening ventricular dysrhythmias, such as torsades de pointes.
will not take nor intend to take concurrently with alprazolam, colchicine, eplerenone, ergot alkaloids (eg, dihydroergotamine, ergometrine, ergotamine, methylergometrine), felodipine, irinotecan, lovastatin, oral midazolam, nisoldipine, pimozide, simvastatin, tolvaptan, or triazolam
will not drink or intend to drink alcohol while on oral ketoconazole as ketoconazole enhances the toxic effects of alcohol resulting in a disulfiram-like reaction to alcohol and enhances the hepatotoxic effect of ketoconazole

understand that because ketoconazole tablets have been associated with serious adverse effects, ketoconazole tablets are not indicated for the treatment of onychomycosis, cutaneous dermatophyte infections, or Candida infections
understand serious hepatotoxicity, including cases with a fatal outcome or requiring liver transplantation, have occurred with the use of oral ketoconazole
understand high doses of ketoconazole may depress adrenocortical function
understand ketoconazole has poor penetration into cerebral-spinal fluid and should not be used to treat fungal meningitis
understand the use of ketoconazole in male patients has been associated with decreased testosterone concentrations
understand female patients with Cushing syndrome treated with oral ketoconazole may experience a decrease in ovulatory disturbances and should be aware of the potential return of fertility
