Zepbound-5MG Injection-1 Box (4 Injectable pens)

Approved on November 8th, 2023, Zepbound is an injectable prescription medication used to help reduce weight for adults that struggle keeping it off. It is indicated for individuals who are overweight, obese, or have medical conditions causing excessive weight gain. It is injected under the skin once weekly and may be increased based on clinical response and tolerance. Follow package instructions upon receiving the product.

I agree that I: 

• am 18 years or older
• have not had a hypersensitivity reaction to Zepbound or any component of the formulation
• do not have a personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN2)
• do not have pancreatitis
  understand this medication may reduce the efficacy of birth control resulting in unplanned pregnancy
• understand that there is limited data about the drug interactions between Zepbound and other prescription medications, herbal products, and supplements
• do not have Type 1 diabetes or diabetic ketoacidosis (DKA)
• am not taking other medications with the same active ingredient or within the same class as Zepbound, including but not limited to, Mounjaro, Ozempic, Trulicity, Saxenda, Victoza, Bydureon or Otezla
• understand serious side effects of Zepbound may include severe stomach issues, kidney failure, gallbladder problems, pancreatitis, low blood sugar, and changes in vision
• am not taking insulin or sulfonylureas, such as glipizide or glyburide, which could increase the risk of low blood sugar
• understand psychiatric effects have occurred, such as suicidal thoughts, with other medications for weight management
• have not had bariatric surgery
• understand this medication may slow gastric emptying which can alter the absorption of other medications
• am not pregnant or breastfeeding
• understand this is not a comprehensive list of precautions, risks, side effects, or details about taking this medication
