I agree that I:  

• am 18 years or older  
• have not had a hypersensitivity reaction to terbinafine, or any component of the formulation  
• have already been using oral terbinafine therapy as prescribed by an authorized healthcare provider  
• have confirmation of diagnostic testing of nail or skin specimens prior to treatment of fungal nail infection as is recommended given potential toxicities of oral terbinafine therapy 
• am not pregnant or breastfeeding  
• do not have chronic or active liver disease 
• do not have active renal disease or impairment 
• understand terbinafine may cause hepatotoxicity ranging from mild and asymptomatic increased serum transaminases to hepatic failure. Terbinafine liver injury may present as hepatocellular hepatitis or cholestatic hepatitis. 
• Understand severe cutaneous adverse reactions (SCARs) have been reported with terbinafine therapy including Stevens Johnson syndrome/toxic epidermal necrolysis (SJS/TEN), drug reaction with eosinophilia and systemic symptoms (DRESS) and acute generalized exanthematous pustulosis (AGEP).  
• Understand altered taste and altered sense of smell may occur with terbinafine and severe cases have resulted in decreased food intake, weight loss, anxiety, or depression  
• Understand thrombotic microangiopathy (TMA), including thrombotic thrombocytopenic purpura and hemolytic-uremic syndrome, have been reported rarely with terbinafine therapy 
• will not take nor intend to take concurrently with doxorubicin, eliglustat, mequitazine, Saccharomyces boulardii, tamoxifen, or thioridazine
