Yuvezzi (carbachol and brimonidine tartrate ophthalmic solution, 2.75%/0.1%) is a fixed-dose combination eye drop approved for the treatment of presbyopia in adults. Presbyopia occurs as the lens of the eye becomes less flexible with age, typically starting around 45, and affects the large majority of adults over that age. Yuvezzi works through two complementary mechanisms: carbachol, a cholinergic agonist, constricts the iris sphincter and ciliary body to narrow the pupil, while brimonidine, an alpha-adrenergic agonist, limits the pupil from widening again and extends how long the effect lasts. Together, these actions create a small-aperture, or pinhole, effect that increases depth of focus and sharpens near visual acuity, generally within about 30 minutes and lasting up to 8 to 10 hours per dose.
Yuvezzi is administered as one drop in each eye, once daily, using a preservative-free single-dose vial that should be discarded after use; one vial contains enough solution for both eyes. Patients who wear contact lenses should remove them before dosing and wait at least 10 minutes before reinserting them, and if other topical eye medications are used, they should be spaced at least 5 minutes apart from Yuvezzi. Because it does not require surgery, Yuvezzi can serve as a first-line or adjunctive option for patients looking to reduce dependence on reading glasses, and it can be used together with corrective lenses worn for distance vision. This medication should be used with caution in patients with a history of iritis, depression, low blood pressure, or circulatory conditions, since the medication's effects on the pupil and blood vessels can interact with these conditions. Counseling should emphasize proper instillation technique, the importance of not touching the vial tip to the eye or eyelid, and awareness that vision may be temporarily blurred or dimmed, particularly in low light.
The most commonly reported side effects of Yuvezzi include eye pain or irritation at the time the drops are instilled, headache, and temporary visual disturbances such as blurry, dim, or dark vision, most of which are mild and resolve on their own. Patients experiencing blurred vision should avoid driving, operating machinery, or performing other hazardous tasks, and should use extra caution with night driving or activities in low light. More serious but rare concerns include retinal tear or detachment, which can occur with pupil-constricting medications like this one, so patients should seek medical attention right away if they notice sudden flashes of light, new floaters, or vision loss. Yuvezzi should not be used by anyone with known hypersensitivity to its ingredients or with active iritis, and it should be used cautiously by people with a history of low blood pressure, poor circulation, Raynaud's phenomenon, dizziness upon standing, or depression, since brimonidine can lower blood pressure and may interact with blood pressure medications or heart-related drugs. Patients should also watch for signs of an allergic reaction, such as hives, difficulty breathing, or swelling of the face, lips, tongue, or throat, and seek emergency care if these occur, and should be careful not to let the vial tip touch the eye or any surface to avoid injury or contamination.
