Tri-Luma® cream (fluocinolone acetonide 0.01%, hydroquinone 4%, tretinoin 0.05%) is a prescription topical medication used for the short-term treatment of moderate to severe melasma (dark patches of skin) on the face. It is the only FDA-approved combination product for melasma and contains three active ingredients that work together:
Fluocinolone acetonide 0.01% – a mild corticosteroid that helps reduce skin inflammation.
Hydroquinone 4% – a depigmenting agent that decreases melanin production to lighten dark patches.
Tretinoin 0.05% – a retinoid that increases skin cell turnover, improving penetration of hydroquinone and enhancing skin renewal.
This triple-combination therapy provides a more effective approach than single agents alone for improving hyperpigmentation.
How to Use:
Typically prescribed for once-daily application at bedtime.
Apply a thin layer only to affected areas of the face, avoiding eyes, mouth, and open or broken skin.
Wash hands before and after application.
Use only the amount directed; applying more will not improve results and may increase irritation.
Treatment is generally limited to 8 weeks, as prolonged use increases risks of side effects.
Important Notes and Warnings:
Common side effects include mild redness, burning, peeling, or dryness where applied.
Hydroquinone may rarely cause ochronosis (blue-black skin discoloration), especially with long-term use.
Because Tri-Luma contains a corticosteroid, extended use can thin the skin or increase local side effects.
Increases sensitivity to sunlight; daily sunscreen use and protective clothing are strongly recommended during treatment.
Not for use during pregnancy or breastfeeding unless specifically approved by a healthcare provider.
Patients should not use Tri-Luma continuously long-term; it is intended for short-term cycles under provider supervision.
Tell your healthcare provider about all other topical products you are using, as combining with other retinoids, corticosteroids, or bleaching agents may increase irritation.
